Forms Key


1. VA Form 22-1990: Application for Education Benefits. To be used for new claims.

2. DD Form 214-Copy #4: Report of Separation Form. This form was issued to you upon discharge from active duty. This form is to be used with new claims.

3. VA Form 22-1995: Change of School or Place of Training Form. To be used when a student has previously received VA Education Benefits at another school and transfers to UConn or a UConn student changing programs at UConn.

4. DD Form 2384: Notice of Basic Eligibility (NOBE) for the Reserve/Guard GI Bill. This form is issued to member from his/her Reserve or Guard Unit.

5. VA Form 28-1905: Disabled Vet Authorization for School Attendance Form. This form is issued from the VA Counseling & Rehab. office directly to the Vet and the School.

6. VA Form 22-5490: Application for Survivors and Dependents' Educational Assistance Form. To be used for new claims.

7. VA Form 22-5495: Change of Program/Place of Training for Survivors and Dependents' Educational Assistance. To be used when dependent or spouse previously received VA Education Benefits at another school and transfers to UConn or changing programs at UConn. This also includes transferring from one branch to another one.

8. UConn VA Request for Benefits Form "D". These forms can be obtained at our office.

9. All appropriate forms should be submitted in a timely manner (see next section on Advance and Regular Pay) to the Veteran Counselor on your campus
